ADRF,Inc, will not be liable or responsible for  the actions of rental car providers, cruise ship providers, airline providers, hotel provider and/or anyone's actions. The awarded recipient agrees to pay for Any/All Services  

received /not  include within the  Respite Wish Package. The Antwan Dominique Reese Foundation, Inc. is Only Liable / Responsible for providing  the items stated within the Respite Wish Package. 

 

The Antwan Dominique Reese Foundation, Inc. disclaims any/all conditions for receiving financial awards. Monetary gifts are not included within the

Respite Wish Package. A photo / statement release  is  required to receive the  Respite Wish.

 

All  respite wish award participants and their spouse/or guest agree and will confirm in writing that :

(a) They will hereby waive all rights to claim personal/bodily injury, They will hereby waive all rights to claim punitive, incidental, consequential, or any other damages against The Antwan Dominique Reese Foundation, Inc., if  they accept the respite  wish package.

(b) Upon receipt of the application, if  the requested information is not furnished completely and accurately, The Antwan Dominique Reese Foundation, Inc. will be unable to  award the respite wish package to the participant.

 

The Applicants disabled youth and/or adult must be receiving services from State of New Jersey (DDD) The Division of Developmental Disabilities* to receive the Respite Wish Package. ( proof of eligibility is required ).

The Student / Applicant must be receiving services from State of New Jersey (DDD) The Division of Developmental Disabilities* and/or Social Security Disability in the State of New to receive  the Herbert Haynes, Sr. Scholarship ( proof of eligibility is required ).

(c) All causes of action arising out of / or connected with receiving this/any award from 

The Antwan Dominique Reese Foundation, Inc., ie: Any/All hotel charges/fees, car rental accrued beyond the Respite Wish Package, destroying  any property, gambling, fighting, indecent  exposure, public harassment, incarceration, alcoholic/drug         

(influenced / induced and wrongful death incidents shall be resolved  individually, and at the cost of the recipients, without resort to any form of class action suit and/or individual suit toward The Antwan Dominique Reese Foundation, Inc.

 

**The recipient is required to accept ALL terms and conditions of this disclaimer to receive the Respite Wish.** A photo / statement release & signed disclaimer release is required to receive the  Respite Wish.
